Medical Information Cards offer a quick, easy way to keep track of medications and conditions for doctor appointments and emergencies. Simply fill in medications and conditions and then fold card to credit-card size to store in wallet for quick reference. One side features a prompt for the date, and eight lines for medications, dose and frequency. The reverse side offers prompts for more medication information with boxes to check for over-the-counter drugs, herbals, vitamins, inhalers, eye drops and supplements.
